Procedure for when there's no discovered cache.
I'm just wondering what you guys do when a cache hasn't been discovered as I don't really know what to do when it comes to this part of Insurgency when I'm squad leading.

Find somewhere you can set up and farm kills, the more kills you get the more intel your team gets meaning the Cache will reveal quicker. This or speak to your commander and get an idea of where the enemys are spawning (possible cache) move towards that area.

I usually dig up in defence. There's no point runing around and getting killed because you don't even know where are you going.
Stay inside some building and kill any rebel scum, that you see, that way you'll gain points for intel

Fizzco is right about needing to get kills. Setting up ambushes and waiting won't be as effective as actively seeking out the enemy. So you're job is to find the enemy. You can do this by splitting up your squad mates in one man teams, and making sure they are in different grids. Having transportation would be ideal.
If transportation is limited, you can group up some of your teams. Just recon but don't engage. Keep the transports alive. Once you find the enemy, regroup and get a spawn point up then attack on foot together as one squad.
Another option is to limit the enemy advance towards likely cache areas. If there are any bridges you can destroy or limited access points into a likely cache area - camp it. If the map is wide open then don't do it as they have a 100 ways into a potential future cache area.

You can't necessarily seek out the enemy as an only tactic. A good insurgent team will try and reduce their presence to stop you getting intel, and thus forcing you to come to them. This is why you can not either sit in base, or seek out the enemy. You need to go to a defensible point in a position that will coax the enemy out of hiding and give you intel points. Never go on a direct offensive as blufor without any caches. Find a good position, fortify it and farm kills as Fizz said (cause he is amazing <3) = GG blufor.
However, if they are a good team, the insurgents will remain in hiding and force you to come for them. Then, you must move carefully around the map to try and engage the outer pockets of the insurgents defense. This is how you get the best insurgency round; when you get a coordinated and patient Insurgent force and a coordinated and slow on the offensive blufor team.
Think of blufor tactics like ancient Roman ones. The legion in line, an extremely defensive force, would run up very close to the enemy battle line and stop and attempt to coax an attack. If the enemy line was counter charging the Romans would simply stop first.
